Insurance is not accepted; however, I am happy to provide you with a statement. Please check with your insurance provider prior to making an appointment and ask if they accept out-of-network services.
Cancellation Policy
Please notify me within 24 hours to cancel your appointment to avoid being billed the full cost of the session.
This fee may be waived in the event of an emergency.
Good Faith Estimate
Under the law, health care providers are required to provide self-pay patients with a "Good Faith Estimate" of cost for treatment. This regulation attempts to provide patients with transparency regarding their health care costs. You have the right to receive an estimate of expected charges for any non-emergency items or services before starting treatment.
